Responsibilities
- Direct and oversee home health services delivery, ensuring
diagnosis-specific patient care
- Manage patient care services and personnel, including
assignments, referrals, assessments, and care plan updates
- Coordinate services with providers, vendors, and
facilities
- Ensure timely initiation of patient care per regulatory
guidelines
- Oversee physician orders, staff assignments, and team
communication
- Review and approve care plans and integrate physician
ordersQualifications
- Education: Associate's Degree in Nursing (required)
- Experience:
- Minimum 2 years as a Registered Nurse in home health or
behavioral health
- Minimum 1 year in a supervisory role
- Licensure: Active RN license in the state
- Technical Skills:
- Familiarity with enterprise applications (e.g., Workday,
HomeCare HomeBase)
- Proficiency in Microsoft Office (Word, Excel, Outlook) We're
