Medical Director | Gilboa, NY Area We are seeking a Primary Care
Associate Medical Director to join our team near Gilboa, New York,
providing comprehensive patient care and clinical leadership in
skilled nursing facilities (SNFs). This full-time role offers a
flexible 4-5 day workweek. Role Highlights: Clinical Care: Conduct
primary care rounds, manage chronic diseases, and implement complex
care plans in post-acute settings. Leadership: Provide oversight
and mentorship to Nurse Practitioners. Quality & Collaboration:
Lead clinical case reviews, collaborate with facility leadership,
and develop care coordination initiatives. Relationship Building:
Foster strong relationships with patients, families, and the
post-acute care community. Qualifications: Minimum of one year of
primary care experience required. Active and unrestricted New York
Medical License (or one in progress). Active and unrestricted
DEA/prescriptive authority (or application in progress). Experience
