matters to us. 1115 Waiver Enhanced Navigator – Catholic Charities
of Fulton and Montgomery Counties The 1115 Waiver Enhanced
Navigator plays a crucial role in supporting individuals as they
navigate available services, helping them access life-changing
resources and empowering them on their journey toward wellness and
stability. In this role, you will: Conduct screenings with
Medicaid-eligible individuals using the approved screening tools —
ensuring accuracy, compassion, and proper documentation. Enter and
maintain timely, accurate data in the Unite Us platform, meeting
all 1115 Waiver requirements. Educate clients, referral partners,
and community members about waiver eligibility and available
services. Participate in required trainings, including Unite Us,
trauma-informed care, HIPAA, and compliance. Collaborate with case
managers and internal teams to deliver coordinated, client-centered
care. Position Details Salary: $21.00/hour Schedule: Monday–Friday,
35 hours a week Qualifications Associate’s Degree in Human Services
or a related field 1–3 years’ experience in mental health,
